Our Site does not host its own appointment booking form or patient intake form. When you book an appointment, you are redirected to Dentrix Ascend(bookit.dentrixascend.com), an independent dental practice management platform. Any personal or health information you submit there is governed by Dentrix Ascend’s own privacy policy.

Similarly, if you enroll in our in-house dental savings plan, you are directed to DentalPlan.me(dentalplan.me/practice/kbsmiles). Information submitted there is governed by DentalPlan.me’s own privacy policy.

b. Information Collected Automatically

Like most websites, our hosting infrastructure may automatically record standard server log data when you visit the Site. This may include your IP address, browser type and version, pages viewed, time and date of your visit, and referring URL. This information is used solely for security monitoring, diagnosing technical issues, and understanding aggregate traffic patterns. It is not used to identify you personally.
